Payment Policy
Payment is Expected at the Time of Service
Payment is required at the time services are rendered unless other arrangements have been made in advance. This includes applicable co-insurance, deductibles, and co-payments for participating insurance companies. Legacy Pediatrics accepts cash, personal checks, Visa, Master Card, Discover card, and American Express. All payments are due at the time of the visit. There is a $25.00 billing fee added if you do not pay at the time of the visit.
There is a service charge of $25.00 for returned checks.
Outstanding Balance
A $25.00 service fee will be added to all bills that have an outstanding balance of 60 days overdue. We realize that people have financial difficulties. Please communicate with our billing and collection staff so that they may assist in creating a financial plan with you. Accounts that are referred to a collection agency may no longer be patients in our practice.
Insurance
We bill participating insurance companies as a courtesy to you. You are expected to pay your co-pay or deductible at the time of service. Please contact our billing office if you have any questions about what insurances we do and do not participate in.
Legacy Pediatrics does not participate with Medicaid or Tricare.
Billing Questions
If you should need any assistance or have questions, please contact our billing department between 9:00 am and 4:00 pm Monday through Friday by calling the regular office number and selecting option 6.
Refunds
Missed Appointments/Cancellations
Missed appointments represent a cost to us, to you, and to other patients who could have been seen in the time frame set aside for you. If you are unable to keep an appointment, please give as much notice as possible. We charge the cost of the visit for any appointment canceled within 24 hours, and we also charge the cost of the visit for any scheduled appointment that is missed. Repeated missed or late cancellations of scheduled appointments may result in discharge from the practice.
Forms
After your child’s physical examination, the daycare and/or school form will be sent to your child’s portal. It will be located in the “Your messages” in the inbox. It is your responsibility to provide the forms to schools, daycare, or camps. If we are asked to send a form for you, there will be a $15.00 charge to do so.
Any additional forms that we need to complete such as family medical leave forms, paid family leave forms, or OPWDD forms, there will be a $15.00 charge, and it will take 3-5 business days to complete.
Credit Card Payments
In New York State, medical debt cannot affect your credit. However, if you pay your medical bill with a credit card and that credit card goes into default, your medical bill can affect your credit. Once you pay a medical bill with a credit card, it is no longer considered medical debit and is subject to affecting your credit.
